Process for Forms and Order of Dates

 

 

Process for Completing Forms

  1. The student's topic is approved by the teacher.

  2. The student fills out the Student Checklist (Form 1A) and research plan.

  3. The Approval Form (1B) is signed by the student and a parent.

  4. The Qualified Scientist and/or Designated Supervisor signs the required forms as needed (Forms 2, 3, 5A, 6A, and 6B).

  5. Other forms are completed as needed:
    • Vertebrate Animal Form (5A or 5B),
    • Potentially Hazardous Biological Agents Form (6A),
    • Human or Vertebrate Animal Tissue Form (6B),
    • Continuation Form (7).

  6. The student meets with the teacher to review all of the above; the teacher fills out the Checklist for the Adult Sponsor (Form 1) and signs it when the project is approved.

  7. The Research Plan, with all appropriate forms, is sent to the SRC/IRB for approval if required.

  8. The student may start the experiment!  (The Actual Start Date of the Project should be recorded to use on Form 1A)

  9. The Regulated Research Form (1C), if required, is signed by the Qualified Scientist AFTER the project is finished.

 

 

Order of Dates on Forms

 

  1. Approval Form (1B) signed.
  2. Other certification forms (2, 3, 4, 5A, 6A, 6B and/or 7) signed if needed.
  3. Checklist for Adult Sponsor (1) signed.
  4. SRC/IRB approval if needed
  5. Actual Start Date of Project
  6. Institutional Research Form (1C) signed (after experimentation)
